Coalition Creates More User Friendly Advance Directives Form
In 2008, the Coalition created a new, user-friendly advance directive form. It allows users to designate a health care agent (sometimes referred to as a medical power of attorney or health care proxy). When individuals cannot - or prefer not to - make their own medical decisions, the health care agent is the person who make these decisions. Please refer to Forms page to access this form in English and other languages.
Coalition Participates in National Healthcare Decisions Day
On April 16, 2008 the Coalition participated in the country's first National Healthcare Decisions Day. Over 300 people participated in nine free workshops the Coalition sponsored with host organizations. During these workshops, medical and legal experts discussed the basics of advance care planning and introduced new documents created by the Coalition.
