LEADERSHIP

The Coalition represents perspectives along the continuum of care for individuals and families facing serious and advanced illness; from diagnosis through treatment and at end of life. Stakeholders include representatives from public, private and non-profit medical, legal and social service organizations as well as patients, caregivers and volunteer community members.

The Coalition is a collaborative body co-chaired by a representative of County government and a leader of a local palliative care or end of life community based agency. Coalition members serve on committees that develop and implement annual workplans in the areas of advance care planning, community based palliative care and education, information and advocacy.
